Yesterday I was running trail when I took a bad step, stumbled/strided badly.  Felt an "ouch" in my knee but was able to regroup and finish the short run (30 min) with no discomfort.  However, within 20 minutes after stopping I could feel a sharp pain on the outside of my knee.  By nighttime it was almost nauseatingly painful.
I have full range of motion in the knee joint (not without pain, but I can bend).  It hurts most to extend the knee but also to flex.  
Everything I read suggests IT band syndrome, but IT band is reportedly an "overuse" chronic injury.  Anyone heard of IT coming on so suddenly?  I have NEVER had lateral knee issues before.  And if so, how long before the acute pain subsides?

I can only speak about the IT issue I had last year.  I am certainly not educated to the whole range of IT problems.  
 
My IT pain would only come about during a run and progressively at that.  It would go away when I stopped running or shortly thereafter.   Sounds like your pain got worse after you stopped running which would lead me to GUESS  that it isn't your IT.  Once you stop running the friction causing the pain shouldn't be present.   Good luck!!

Paul,
Interesting.  For lack of anything better to do I tried some IT stretches last night then hobbled around the yard for 30 minutes.  After 20 min I noticed the pain was reducing and this am it is GONE.   Grin  I will take a few more days off and then try a light run and see if the pain comes back during running.  Fingers crossed...

My experience was exactly like Paul's, so I would agree with his assessment.  Hopefully it's a non-issue after this morning's lack of pain.

I had something like this just prior to a half-marathon; I had the call of nature and so went out into the woods and took a very bad step.  My symptoms were similar to yours.
 
I was fine within a week.

OK, lateral knee pain definitely went away with stretches and rest.  Did a light run Friday, 3 mi (average pace and distance for me ) on road Sat, all was fine.  Monday did a slower pace on trail for 30 min with no pain, but as soon as I picked up the pace the pain came back.  So I am thinking it is IT.  Those of you who said they had experienced this, any suggestions on how to make the pain go away and stay away?  (Short of quit running  undecided )

i'm no authority, but i'd have to say rest, stretching (cross-legged hang works well for me), and a slow build-up of mileage.  haven't had it troubles in years and i'm thinking mine was from overuse, too much too soon.  some people swear by those roller massage things.

Thanks, dirtdevil.  Have been doing lots of cross-legged hanging and yes, it does seem to help.  And yes, I know, rest -- why can't these things happen in winter not the middle of the racing season ??   undecided  I really don't think this is due to overuse because it came on sooo suddenly after the bad step, and I really haven't upped my mileage or pace dramatically in the past month.  Which is what makes this so frustrating.

Just read your original post. I had something very similar happen back in February. Stumbled on a root and wrenched my knee. It was fine walking and running for the first few miles then the outer knee pain would come on suddenly and would be excruciating on the downhills (as in crying, alone, up against a tree in the middle of Farmdale in pain and frustration). It would radiate up the outside of my thigh and feel so weak I wasn't sure my leg would support any weight. Uphills were fine, though.
 
Turns out I pinched the plica tissue inside my knee. No structural damage from continuing to run, the doc said, but it would hurt like heck until it healed. He even gave me the go ahead to keep training for the 50-miler. It took until probably mid-April before I felt good-as-new, but no problems now.

And, no, I won't admit to which doctor told me it was OK to train for an ultra with an injured knee. He did know I'm friends with Ann, so he probably assumed I would do it anyway.  Grin Grin

Darn LG I was hoping you would say which doc it was, so I could make sure to go to that one ...
I will research plica tissue.  What you describe is pretty similar to what I feel.  Definitely hills and stairs.
